What specific problem are you solving?
Despite the availability of a cure, over 4 million people in the U.S. live with hepatitis C, and most remain undiagnosed or untreated. The burden is especially high in low-income, rural, and justice-involved communities—many of whom receive care at Federally Qualified Health Centers (FQHCs). These safety-net clinics are overwhelmed and often lack the capacity to track patients through a fragmented care process that requires labs, prior authorizations, and specialist referrals. Even when patients test positive, fewer than 1 in 3 receive curative treatment. This gap represents a massive missed public health opportunity: hepatitis C is one of the only chronic diseases we can cure with a short, effective course of pills. Yet in the absence of technology-driven outreach, strong care coordination, and clinical infrastructure, this promise is not reaching the people most affected.
What is your solution?
EqualCure is building a nonprofit platform to help safety-net clinics find, diagnose, and cure patients with hepatitis C. Our solution includes three core components: (1) a machine learning algorithm to identify patients at high risk of hepatitis C who have never been screened, paired with an EHR registry of patients who have tested positive but remain untreated, (2) a care navigation program to help patients complete screening, diagnostics, and procure medications, and (3) direct access to clinicians who can prescribe cures via telehealth. Community health centers benefit from medications dispensed in their pharmacies via the 340B drug discount program. Our platform will integrate with EHRs to streamline provider workflows, deliver algorithm-informed patient lists, and offer real-time clinical support to help primary care teams confidently prescribe curative therapy. EqualCure’s goal is to make it simple—and financially feasible—for clinics to cure every eligible patient in their care.
Who does your solution serve, and in what ways will the solution impact their lives?
EqualCure serves patients who receive care at Federally Qualified Health Centers (FQHCs), with a focus on people who are uninsured, publicly insured, low-income, or impacted by incarceration, housing insecurity, and substance use. These are the populations most likely to be living with untreated hepatitis C—and least likely to benefit from advances in treatment. While a cure exists, the journey from screening to treatment remains fragmented and nearly impossible to navigate without support. EqualCure bridges this gap by helping clinics identify at-risk patients and guiding them through every step of the process, from diagnosis to cure. Our solution is designed to reduce delays, remove barriers, and build trust—ultimately transforming a long-neglected disease into one of the few curable public health conditions for the communities most affected.
