What specific problem are you solving?
Maternal mortality remains unacceptably high in India, with over 30,000 preventable deaths each year. Among the 30 million women who become pregnant annually, many face undetected or underestimated risks due to gaps in early identification of high-risk conditions and sustained follow-up. Timely, personalized care is critical, but health systems often lack the tools to prioritize which women need urgent attention. Traditional risk assessments—like paper-based checklists or risk flags—are static and reactive. They often detect high risk only after complications arise, missing opportunities for prevention. These tools also overlook the complexity of maternal risk, which is diverse (over 40 potential risk factors), interrelated (mild conditions can combine to escalate overall risk), and dynamic (risk profiles change over time). Frontline health workers are forced to rely on fragmented data and subjective judgment, limiting their ability to provide targeted care. There is a clear need for a solution that enables real-time, risk-stratified decision-making tailored to each woman’s evolving context. PROS addresses this need by using AI to predict risk trajectory and deliver timely, actionable insights to providers. This enables a shift from reactive care to proactive data-driven maternal care, integrated into public health workflows, and scalable across diverse, underserved geographies.
What is your solution?
PROS (Predictive Risk Stratification) is an AI-powered maternal health solution developed by Jhpiego in partnership with Wadhwani AI. It enables frontline health workers to identify and support pregnant women at the highest risk of complications early, accurately, and continuously. The solution uses routinely collected antenatal care data, such as vital signs, medical history, and sociodemographic factors to generate a personalized, dynamic risk score for each woman. These scores evolve in real-time as new data becomes available, reflecting the shifting risk profile across pregnancy. Trained on large, anonymized datasets covering the full continuum of care, from early pregnancy to labor and delivery, PROS currently predicts the likelihood of women missing future antenatal visits. It is now being expanded to detect clinical risks for both mothers. Risk alerts and risk stratified due-lists are embedded directly into the service delivery app already used by frontline workers. These alerts are simple (color-coded) and paired with decision-support prompts, helping workers take timely, appropriate actions. By enabling risk-stratified, proactive care and integrating seamlessly into existing public health workflows, PROS empowers health systems to prioritize better, intervene earlier, and ultimately save more lives.
Who does your solution serve, and in what ways will the solution impact their lives?
The PROS solution serves three key stakeholders: A. Pregnant women in underserved communities: Many face compounded risks due to poverty , limited access to care, and delayed detection of complications. With over 30 million pregnancies annually in India, the burden on the system makes personalized care difficult. PROS helps ensure that the highest-risk women are identified early and receive timely, prioritized care—potentially preventing life-threatening complications for both mother and baby. B. Frontline health workers (auxillary nurse midwives - ANMs, nurses, midwives): Tasked with high caseloads and limited tools, they often rely on subjective judgment or static checklists. PROS provides real-time, dynamic risk alerts integrated into their existing workflow, enabling smarter prioritization, better decision-making, and more efficient use of their time and resources. C. Public health managers and government stakeholders: Managing large maternal health programs at scale is challenging, especially with limited data visibility. PROS generates actionable, population-level insights to inform targeted supervision, resource planning, and improved program outcomes. Currently being deployed across select districts in the central state of Madhya Pradesh in India with plans for multi-state deployment, PROS is designed to strengthen maternal care delivery where it's needed most, at the last mile.
