What specific problem are you solving?
Tuberculosis (TB) is one of the world’s leading causes of death from a single infectious agent. In 2023 in Mozambique, an estimated 121,000 people contracted TB—equivalent to one person every four minutes—and approximately 7,700 people died from the disease (WHO, 2023). The national TB prevalence stands at 361 cases per 100,000 people. This crisis is closely tied to socio-economic challenges. About 62% of Mozambique’s population—roughly 21 million people—live in extreme poverty (Statista, 2024), which limits access to healthcare and contributes to delayed diagnosis and treatment. The country has a high HIV prevalence (12.5%), further increasing vulnerability to TB. Stigma is another powerful, yet often overlooked, barrier to care. Globally, it discourages people from seeking timely diagnosis and treatment. In Mozambique, our Community-Led Monitoring (CLM) initiative, conducted between 2022 and 2024 across three provinces, identified stigma as the most frequently reported obstacle to TB care. Of 9,751 reported barriers from 20,828 people with TB (PWTB), 38% (3,803) were related to stigma. Without addressing stigma and related barriers—such as long travel distances, limited access to health facilities, and financial hardship—it will be impossible to improve early diagnosis, timely treatment initiation, and overall TB outcomes.
What is your solution?
Our solution is to empower PWTB to actively participate in identifying and resolving barriers to TB services through CLM using the OneImpact digital platform. OneImpact is a mobile- and web-based platform that enables real-time reporting of challenges faced by PWTB. It generates data and supports the community response through more efficient data collection and analysis. It enables CLM implementers to better identify challenges and offer actionable solutions and evidence-based recommendations to improve the effectiveness, efficiency, and equity of health services. It also promotes the availability, accessibility, acceptability, and quality of TB care and support services. It consists of three key components: 1. Community App – A mobile application used by PWTB or with support from community health workers. It provides TB and human rights information, allows users to report barriers, connect with peers, and participate in surveys. 2. Response Inbox – All reported barriers go to this inbox, where trained “First Responders” direct them to the appropriate authorities. Coordination meetings organize follow-up actions. 3. Accountability Dashboard – A real-time dashboard with infographics showing trends and key indicators. It helps health facilities and partners monitor progress and improve services. This system strengthens community engagement and drives local accountability.
Who does your solution serve, and in what ways will the solution impact their lives?
Our human-centered CLM solution focuses on PWTB, especially those living in extreme poverty. TB disproportionately affects underserved populations—particularly individuals in remote areas with limited access to health services. In addition to geographic barriers, PWTB face challenges such as inadequate service delivery, stigma and discrimination, and lack of access to accurate health information. The OneImpact digital platform empowers PWTB to safely and anonymously report barriers they encounter—such as service gaps and stigma—without fear of repercussions. It allows users to access information, connect with peers, and raise concerns. For individuals with limited digital literacy, an assisted model is used, where trained OneImpact activists collect data offline. Real-time data is analyzed by project staff, healthcare workers, and local officials to inform timely, targeted responses. From 2022 to 2024, 20,828 PWTB across three provinces used the platform. Stigma emerged as the most reported issue (38% of 9,751 cases), and targeted interventions led to a 72% resolution rate of stigma-related cases. In total, over 85% of all reported barriers have been addressed locally—demonstrating the platform’s power to improve access, equity, and quality of TB care for the most vulnerable.
