What specific problem are you solving?
We are addressing the growing global challenge of managing chronic conditions and ensuring the safety of aging adults, particularly in home settings. Worldwide, over 1 billion people are aged 60 or older—a number expected to double by 2050. In Europe and North America alone, 80% of older adults live with at least one chronic disease such as heart failure, diabetes, or COPD. These conditions often lead to preventable hospitalizations, reduce quality of life, and burden healthcare systems and caregivers. In many communities, especially rural or underserved ones, regular clinical monitoring is inaccessible. Family members and caregivers are overwhelmed and lack real-time tools to track their loved ones’ health. Healthcare professionals are stretched thin, with limited data to proactively manage patients outside the clinic. Cardiomo solves this by enabling continuous remote monitoring of vital signs combined with AI-powered alerts and digital assistant functionality, ensuring timely intervention. By connecting patients, caregivers, and clinicians in real time, Cardiomo reduces hospitalizations, improves care coordination, and empowers aging adults to live independently longer.

Cardiomo is an AI-powered remote health monitoring and digital assistant solution designed to keep aging adults and chronically ill patients safe at home. The system includes a small, non-invasive wearable device placed on the chest that continuously tracks vital signs such as heart rate, respiratory rate, temperature, and activity. This data is analyzed in real time by AI algorithms to detect early signs of health deterioration. In addition to health monitoring, Cardiomo includes a digital assistant that provides health reminders, connects patients with caregivers or clinicians, and delivers personalized recommendations. When an abnormality is detected, alerts are sent instantly to family members and healthcare professionals via mobile and web apps. The solution uses IoT, cloud computing, and machine learning to enable continuous, proactive care, reduce unnecessary hospital visits, and support independent living. Cardiomo is easy to use, even for non-tech-savvy users, and is currently being piloted in partnerships with healthcare providers and care facilities.

Cardiomo serves older adults, particularly those living with chronic health conditions, and their caregivers, including family members and healthcare professionals. This population is often underserved due to limited access to continuous care, especially outside hospital settings. Many older adults live alone or in areas with limited healthcare infrastructure. They are at high risk of complications due to delayed detection of health issues, often resulting in emergency hospitalizations that could have been prevented. Caregivers—both professional and informal—face challenges in monitoring patients 24/7, leading to burnout and gaps in care. Cardiomo addresses these needs by offering real-time, continuous remote monitoring of vital signs and health status, combined with AI-driven alerts and a digital assistant that ensures timely interventions. It empowers older adults to live safely and independently at home, gives peace of mind to families, and helps healthcare providers reduce unnecessary admissions and improve patient outcomes. By transforming reactive care into proactive, preventive care, Cardiomo directly improves quality of life, safety, and health equity for a vulnerable and rapidly growing segment of the population.
