Pakistan ranks 154th out of 195 countries in terms of quality and accessibility of healthcare. We face challenges both on the supply side (availability of appropriate healthcare resources) and the demand side (perception and use of available healthcare resources). The situation is further worsened by COVID-19, as outpatient clinics have been closed with healthcare staff being redirected to emergency and isolation wards. Sehat Kahani has been the first respondents to deliver health services through our mobile application and 24/7 helpline for beneficiaries to be screened and referred for COVID-19; while providing a remote platform for medical professionals to deliver healthcare without the risk of exposure. Globally it has the potential to contain the pandemic by integration of relevant epidemiological data and geographical information that will allow the tracing of cases as well as provide a safe platform for health professionals around the world to connect with their patients remotely.
Pakistan is a developing country with a weak healthcare system that failed to deliver a prompt response for prevention and control of COVID-19. Firstly, with over 207 million inhabitants and only 25 diagnostic laboratories with limited capacity (1700 tests per day on average), the population of Pakistan are becoming potential carriers, particularly because of the asymptomatic transmission of infection and a lack of proper surveillance.With a literacy rate of only 59%, there is a huge sum of people who are still unaware of the situation due to which they are vulnerable and at risk of COVID-19. The situation is further worsened due to lack of female doctors as 77% of female doctors in Pakistan discontinue from practicing medicine due to socio-cultural barriers; further weakening the healthcare system of Pakistan. On the demand side, due to the alarming increase in rates of COVID-19, the patients/consumers are becoming more discerning around their own health and customer experience has become a key motivator for providers. Healthcare providers are failing to adapt to this consumer shift due to lack of remote platforms. Prioritizing resources is especially relevant as Pakistan looks to mobilize innovative solutions for the COVID-19 response.
Sehat Kahani has developed a mobile health application that allows health coverage in the absence of traditional alternatives due to the current pandemic. Through this application, users can obtain real time video consultations with general and specialist female doctors along with a complete cloud based electronic medical records system that keeps track of their health history and store and forward digital prescriptions, e-lab referrals and consultation summaries. Using their smartphones the solution can be utilized by beneficiaries to avail general consultation for their ailments or to self screen for suspected COVID, receive health information and referrals for COVID-19 as well \as avail treatment services for mild COVID cases. Our mobile application provides access to a wider population at low cost that is helpful in delivering healthcare interventions, without requiring people to visit health services physically. We have also established a 24 × 7 support center using a helpline number that provides emergency medical response or medical advice using tele-triage mode. The helpline is currently also functioning as a self-screening and referral tool for users who are suspected for COVID-19. The application is also ensuring feasibility for healthcare professionals to connect to their patients remotely without the risk of COVID exposure.
At demand end our target beneficiaries are the general population that might not have the accessibility to healthcare services due to nationwide lock down and closure of outpatient clinics. We will also be extending our solution to corporate and large organizations so that they can either provide access to our application to their employees or to their clients. On the supply end our beneficiaries are the female doctors that do not continue their medical practice due to social pressures, cultural taboos and lack of support from family. Sehat Kahani approaches the challenge of converting need into demand through a human-centered design approach. Our network of doctors are trained to deliver quality health services while incorporating feedback from our beneficiaries to ensure shared decision making and informed health choices are being made. Through these techniques, Sehat Kahani provides a service that women value, building a demand for low-cost, customer-centered healthcare that has translated into a greater likelihood of patients availing remote services for their ailments or for suspected COVID cases. Our solution is also paving the way for unemployed female health resources to continue their medical practice remotely while building their capacity to utilize mobile based solutions in their practices.
With the increasing capacity of health professionals needed globally to reach Sustainable Development Goal 3 on health and wellbeing by 2030, our mobile based solution can support healthcare workers and the broader health systems by empowering nurses, midwives, doctors and other health workers. Our technology has the potential to streamline the triage, diagnoses, and post-care follow up process, thereby stretching health workers’ limited time to serve more patients. Our solution can also be used to better understand patterns in the spread of diseases, like the case of COVID-19 thereby containing the further spread of infection while strengthening our healthcare system.

Our technology is a complete end to end solution for our beneficiaries that include features such as: doctor’s profile with appointment slots booking, complete patient EMR system, video-consultation, chat to doctor, e-prescriptions and e-labs. We use real time data analytics to find disease patterns utilizing data that was never captured before. We use Power BI in analysing patient data that is automated into real time dashboards to understand existing health needs of our beneficiaries. In addition for data storage we utilize the Microsoft Azure cloud services, along with its compliance to HIPAA standards to ensure patient data protection. In addition, our technology is integration with SMS APIs that enables automatic SMS reminders to our beneficiaries with follow up appointment details.For follow up visits that do not require physical visit to clinics, beneficiaries can utilize our mobile application that is widely available on iOS and android app stores. Through the mobile application these beneficiaries can avail virtual consultations,access counselling services as needed and view/download their labs, scans, e-prescriptions, referrals and health education material remotely. We also aspire to integrate machine learning in our existing technology to improve user engagement through AI integration in order to analyze user behavior to provide insight into the user. We will utilize this information to make changes for better user engagement. In addition, by integrating chat bots, predictive algorithms and real time automated dashboards, beneficiaries utilizing our solution will be able to make a well-informed decision.

Mediktor has launched a Coronavirus assessment self-test using mobile application. It is online and free with a target reach of 49.6M beneficiaries. It provides a differential diagnosis and triage tool, not only checks the disease symptoms but also provides other possible diseases associated with the symptoms.
H-casa is a web app for citizens to use for a first informative self-diagnosis via questionnaire, establishing contact with the family doctor who, if appropriate, will contact him to perform a pre-triage by telephone and, consequently, to activate teleconsultation for the transmission of clinical data from the patient’s home on the platform.

Sehat Kahani; like all the other Global Players in the telemedicine market, intends to grow its current business by expanding their Telemedicine Platform through the mobile application and not only become a market leader in Digital Health in Pakistan but also become a global player. With the current COVID scenario; the inertia towards telemedicine has already been broken and the acceptability of seeking a doctor’s opinion over a video call through a smartphone while being able to maintain social distancing and reducing the risk of exposure by not going to a hospital has grown tremendously. Sehat Kahani has already build a strong network of corporates and industry magnets (Including Banks, Telecoms and Pharmaceuticals as the key giant players) as early adopters to champion Sehat Kahani’s vision and impact during the current COVID Pandemic where they are availing their services for their employees. In the next five years, we aim to be able to lock over 10 million active users availing e-health services through the E-Health application, shifting the trend from physical health towards virtual OPD consultations and other digital diagnostic services and become the next ‘Uber for Healthcare’ in Pakistan. The following challenges and consequences are anticipated for adapting, integrating, and scaling our innovation.
Regulatory Risk: While the benefits of using mobile devices may be easy to see, the fact remains that healthcare is one of the most highly regulated industries. There are a number of regulations that includes consultation guidelines, prescription regulations and payment mechanisms
Reimbursement: While clinicians may be willing to jump on the mobile health bandwagon, they need help from governing bodies to be successful. Much like the regulation issues, the reimbursement opportunities for mobile health are falling behind the demand. Telehealth consultations are often not reimbursed from medical benefits of beneficiaries resulting in out of pocket expenses.
Security: Security in the healthcare landscape is a never ending challenge. The need to avoid HIPAA violations and keep patient information safe has only become more complicated as the use of digital technology has expanded.
IT Support: Mobile devices and telemedicine require more than just the right devices. They also require the right backend support. These devices need a strong and consistent wireless signal to be effective. This means having a facility-wide network that can support multiple kinds of devices and can secure all the information that is shared over it.

We have a four year track record of creating e-health and m-health solutions that has been utilized by over a million beneficiaries. With the current pandemic implications we aspire to develop a solution that is accessible to a wider, eventually global, audience. By being a MIT solver, we want to empower the female health workforce to utilize remote solutions in order to improve health outcomes as well as contain andx prevent COVID in Pakistan. We feel that the MIT community can help us emerge as better leaders for our company as well as support our collective vision of strengthening primary healthcare of Pakistan by integrating m-health solutions. As the world grapples with COVID-19, every ounce of technological innovation and ingenuity harnessed to fight this pandemic brings us one step closer to overcoming it. Artificial intelligence (AI) and machine learning are playing a key role in better understanding and addressing the COVID-19 crisis. To tackle this overwhelming challenge, Sehat Kahani developed a partnership with Microsoft of over 2 years, began with the vision to not only enhance the quality of the patient care but also the patient data collected over the years. With the Sehat Kahani technology, operational in both the low income segment and for smartphone users, the team envisions the company growing exponentially over the next 5 years. To ensure the quality of care and accuracy of the diagnosis, it is essential to involve Data Analytics, Artificial Intelligence and Machine Learning.
Sehat Kahani uses Power BI in analysing patient data currently in the clinics, helping us with the monthly closing and preparation of dashboards to get a bigger picture, and we aim to integrate it in our Mobile App as next steps. Along with that Sehat Kahani currently uses the Microsoft Azure cloud services, with migration of few segments still in process. The motivation behind using Azure is its conformation to global standards, with security and privacy of data being the top most priority, along with its compliance to HIPAA standards to ensure patient data protection which is of utmost importance to healthcare organisations. Furthermore, the service is known as one of the most credible one for Artificial Intelligence hence is in line with our vision of integrating wearable devices for more accurate results, automated monitoring and flagging of disease trends as part of data analytics and shaping the app to make it more personalized.
With the funding, Sehat Kahani will be able to upgrade the solution by integrating clinical algorithms in the application to aid physicians in determining a disease diagnosis. Clinical flowcharts will also be included to help physicians identify diagnostic possibilities. A web bot/ chat bot will be created and integrated so that our beneficiaries can access health related information and if needed healthcare workers can assess and interact with patients via virtual visits. The automated chatbot will also have the ability to auto redirect patients to available online doctors and specialists to receive virtual consultation. By utilizing a pool of qualified female physicians, Sehat Kahani created its state of the art Telemedicine solution which has successfully catered to more than 165,000 patients across Pakistan to date. Clubbed with its health education drives, Sehat Kahani has successfully impacted more than 1,230,000 people across Pakistan.
During the current COVID crisis; An already fractured and fragmented healthcare system in Pakistan is struggling to meet the increasing needs of patients being detected for corona-virus every day while the regular Outpatient departments of hospitals have been shut down. In this time of national crisis we have played a very prominent role to ensure people are able to access qualified healthcare professionals while maintaining social distancing through Sehat Kahani and the providers of all of these services have been female physicians. We strongly feel that because of this; Sehat Kahani definitely deserves the Innovation for Women Prize.
