What specific problem are you solving?
In the pastoralist communities of Samburu County, access to healthcare is limited, particularly for those with chronic conditions like hypertension, diabetes, and asthma. These conditions often go undiagnosed or untreated due to the long distances people must travel to reach healthcare facilities, lack of specialized care, and cultural barriers to seeking medical assistance. As a result, the prevalence of these diseases is rising, and complications from untreated conditions often lead to preventable deaths, disability, and diminished quality of life.
Globally, over 1.5 billion people live with chronic conditions, and according to the World Health Organization, 80% of premature heart disease, stroke, and diabetes-related deaths occur in low- and middle-income countries, where rural populations, like those in Samburu, are most affected. In Kenya, chronic diseases are responsible for 30% of deaths, with rural communities bearing a disproportionate burden.
In Samburu, approximately 70% of the population lives in remote areas, where healthcare facilities are few and far between. This problem impacts thousands, leaving them without proper diagnosis, treatment, and management of chronic conditions. Our solution—telemedicine for chronic care—bridges this gap by providing remote consultations with specialists, ensuring people receive timely care without the need for long, costly travel.
What is your solution?
Our solution, TeleCare for Chronic Conditions, leverages telemedicine to provide accessible healthcare for people living with chronic diseases in remote pastoralist communities in Samburu County. Through a network of trained community health workers and mobile technology, we enable patients to receive consultations, diagnoses, and treatment plans from specialists without needing to travel long distances to healthcare facilities.
Patients visit local health centers where community health workers help them with basic health assessments, such as taking vital signs and uploading medical data to a secure platform. Using mobile phones or tablets, they can then connect to remote doctors and specialists via video calls, voice consultations, or chat. The technology used includes simple, user-friendly telemedicine platforms that are compatible with low-bandwidth internet and mobile networks, ensuring accessibility even in rural areas.
Our solution ensures that chronic conditions like diabetes, hypertension, and asthma are effectively managed, and patients receive the proper care and medication without the burden of travel. This model promotes long-term health monitoring and reduces the risk of complications from untreated conditions.
Who does your solution serve, and in what ways will the solution impact their lives?
Our solution serves marginalized pastoralist communities in Samburu County, Kenya, particularly individuals living with chronic conditions such as diabetes, hypertension, and asthma. These communities are often underserved due to the remoteness of their locations, limited healthcare infrastructure, scarcity of specialists, and high costs and difficulties associated with traveling long distances for medical care.
Currently, many residents delay or forego treatment because of these barriers, leading to worsening health conditions and avoidable deaths. Women, children, and the elderly are especially vulnerable.
Our solution will bring specialist healthcare directly to their communities through telemedicine. By connecting them with doctors remotely, we eliminate the need for expensive and time-consuming travel. Community health workers will assist patients in using mobile technology for consultations, ensuring even those with low literacy can access care.
This will improve early diagnosis, continuous monitoring, and management of chronic illnesses, resulting in healthier lives, reduced medical emergencies, and empowered communities that can thrive without being held back by preventable health challenges.
