What specific problem are you solving?
Amina is 52. She has lived with hypertension and diabetes for over a decade—but without regular checkups, affordable medication, or a clear long-term care plan. Her daughter, her only caregiver, juggles work and debt while managing Amina’s fragmented care. Last month, Amina had a stroke. It could have been prevented. This is not a rare story—it’s a national reality. In Nigeria, chronic and non-communicable diseases (NCDs) like hypertension, diabetes, and cancer account for 29% of all deaths. The probability of dying prematurely from an NCD is 22%. Over 13 million Nigerians live with hypertension, and 125,000 new cancer cases emerge annually, many diagnosed too late. Yet, less than 5% of Nigerians are enrolled in the national health insurance scheme, and care is often disjointed, expensive, and reactive rather than preventive. Key factors driving this crisis include: – Lack of coordinated and continuous patient-centered care pathways – Limited access to affordable medications and health services – No structured support systems for patients and caregivers – High out-of-pocket spending Globally, NCDs are rising rapidly in LMICs, now causing over 85% of premature deaths in these regions. They account for 74% of all global deaths, making this a silent emergency that is both personal and planetary—demanding long-term, inclusive solutions.
What is your solution?
Auxichron is a digital care coordination platform revolutionizing how individuals with chronic and long-term conditions access, manage, and sustain care—especially in underserved communities. Our mobile app consolidates essential health services into one seamless experience. Who does your solution serve, and in what ways will the solution impact their lives?
Auxichron serves individuals and families living with chronic and long-term conditions—those who need continuous, daily care and support to maintain their health. These include patients managing hypertension, diabetes, sickle cell disease, or cancer in underserved communities, where less than 5% are covered by health insurance and care is often fragmented, costly, and inaccessible. For these patients, Auxichron offers a lifeline: a unified digital platform providing personalized care plans, medication delivery, AI-powered adherence tools, support groups, and subsidized access to a network of vetted providers. It transforms isolated, inconsistent care journeys into coordinated, proactive ones—improving outcomes, reducing complications, and lowering out-of-pocket costs. We also serve healthcare providers. Through Auxichron, doctors, pharmacists, and allied health professionals can manage their patients more effectively, track outcomes, and collaborate across disciplines for holistic care. Our platform unlocks additional income opportunities through teleconsultations, referrals, and medication reviews—while reducing the administrative burden and patient drop-off rates that often affect provider efficiency and retention. By connecting patients, caregivers, and providers through smart, scalable technology, Auxichron is redefining what long-term care can look like—structured, supported, and sustainable.
